All athletes must have a sport physical online in Privit before a student can participate.

All student-athlete paperwork including physicals can now be completed via an online system called Privit. Once you complete your Privit profile you will print the physical form directly from this site and take to your physician for clearance.

Ready to get your physical?

  1. Have you completed your Privit profile? If NO complete profile first, then proceed. If YES, proceed through following instructions.

  2. Once you have completed your profile and joined a team click on the PRINT DOCUMENTS tab Print out the MSHSAA - Medical Eligiblity Form

  3. You will then take this to the physician for your physical.

  4. You need only return page 5 of the form (MSHSAA Medical Eligibilty) . You can either scan and upload into your Privit account or bring to school to be uploaded.
